Recently our Marketing Director, Parrialee Holmes, along with her team of devoted stakeholders arrived early to serve coffee, apple cider and doughnuts to case managers from the local hospital called Kennestone Hospital. It was “Drive through for a Brew,” and it was a big hit.
Our purpose was to say thank you to all case managers who play a vital role in the patient’s transition from a hospital to the next care setting. They are pivotal to patients care coordination, with the goal of moving the individual to self-care whenever possible. They address the total individual, inclusive of medical, psychosocial, behavioral and spiritual needs. Thank you to all case managers for empowering people to understand and access quality efficient health care.
For our part, nursing home facilities now play a key role in helping people go home after a stay at the hospital. Many of us find that we need a few days or weeks of extra rehab and care before we are ready for self-care at home.
